    Responsibilities

    • Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history reviews to determine care needs.
    • Administer medications, injections, IV infusions, and perform wound care such as suturing and ostomy management following aseptic techniques.
    • Manage complex care plans involving chronic conditions like diabetes, heart disease, respiratory issues, and neurological disorders.
    • Utilize electronic health record (EHR) system, Axxess, to document patient interactions accurately and maintain detailed medical records.
    • Educate patients and their families on medication administration, disease management, mobility assistance, nutrition, and safety measures at home.
    • Coordinate with multidisciplinary teams including physicians, therapists, social workers, and case managers to ensure seamless care delivery.
    • Provide triage support by assessing urgent needs over the phone or during visits and escalating cases appropriately for emergency or specialized intervention.

    Experience

    • Valid RN license with current state registration or licensure required.
    • Proven experience in home health nursing or related settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics preferred.
    • Familiarity with EMR/EHR systems like Axxess to ensure efficient documentation and communication.
    • Critical care experience including ICU or emergency medicine is highly desirable for managing acute situations.
    • Knowledge of specialized areas such as hospice & palliative medicine, infectious disease care, trauma medicine (Level I & II trauma centers), dialysis support, cardiac catheterization procedures, and long-term care practices.
    • Skills in IV insertion, medication administration via various routes, specimen collection/processing, and infection control protocols.
    • Experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (memory & dementia care), behavioral health patients, individuals with developmental disabilities or disabilities requiring complex case management.
